My name is Candace Reid and I was a consultant for City Impact. My email registered with Jotform was creid@cityimpact.org. Since I am no longer a consultant there, my email address has been deactivated. My personal card was registered to the City Impact account which I am in posession of. The attached document is of the original questionnaire form used to create the original form, Background Profile. I worked at Franchise Building Expert for about 10 yrs and now I work for myself and do not have access to that email. I was just informed that I still had "FBE" logos on my form and I saw my email was still the old one. When I went to change it I needed to verify through that email which doesnt exist any longer.

I am on an American Airlines flight to Italy. I was loading up a Jot form document for a Grant I was applying for. At the very end, I was trying to attach an excel file and lost EVERYTHING. All my work that had been carefully loaded over several hours was lost.
I missed the deadline for the grant minutes later.
if you can find my work it was grant for US Rowing's United We Row program.

I've never had an issue like this before with Jotform. I've had people register, etc and now I can't log in at all. It keeps telling me to reset my password but it's not allowing me to.
