Now, coming back to your question, When I cloned and tested your form, everything worked the way it was supposed to. Take a look at the screenshot below to see my results:

Additionally, please try integrating your form into Discord by following the steps below:

1. In Form Builder, click on Settings in the orange navigation bar at the top.
2. Select Integrations on the left side and click Discord.
3. Click on Connect to Zapier.
4. Log in to your Zapier account and click on Authorize.
5. Next, you need to configure your Zap. Click Choose to choose an account.
6. Click on Continue.
7. Click on Continue again.
8. Click on Test Trigger.
9. Select a record and click on Continue with the selected record.
10. Log in to your Discord account and select an Event.
11. Then click on Continue.
12. Click Continue again in the next step and fill out the fields.
13. Then click on Test Step to make a test.
14. Lastly, click on Publish and you're done.

I cleared your form caches to resolve any cache-related issues. Also, can you make a test submission before integrating Discord? This is to ensure that there are submissions to pull when you make a test trigger in Zapier.
