Hi Arion444,
I verified your issue by cloning your form. I found out that on your Email Alerts Notification, it does not match the field name. Please check the screenshot below.
To resolve your issue, please try deleting this notification and creating a new one. Here is a guide on Setting up Email Notifications in case you need it. It should show you all the default fields from the form that you will received in your email.

This is still not working correctly!! I followed the steps for Email Form Notifications, and the test email I get still has these fields only:

Okay, I think I got it figured out. Please clone and check and tell me if there's anything else I need to do.

Arion444Replied on October 7, 2014 at 9:11 AMI did open a new ticket. Not been answered yet. Has to do with editing text color and other changes for fixed fields. Some of the text is illegible on my Web page. .
