Images in Multiple Choice element shows up as "?"

That's another weird thing is the swatches sometimes show up and like in the screen shot a couple show "?"

I checked the form and everything is working fine. I did not get any "?" on the swatches. See my screencast below:

This could just be caused by a slow internet connection. You can try accessing your form using a different internet service provider or try another browser or device and see if this still persists.
