I cloned your form to test it, and I easily replicated the issue. There is an easy fix for this, we just need to clear the field values when hidden in Form Settings. Let me show you how:

1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
2. Click on Show More Options at the bottom.
3. Scroll down to Clear Hidden Field Values option.
4. Change it to "Clear when hidden".

That's it. Here's the result:
