Changing logo position and printing the form

Thanks for your patience. I’m sorry you're having difficulties with this. When I cloned and tested your form. After investigating it a bit, I found a solution. We just need to add a form logo on the PDF editor. As for the second page being blank, I do see that there is only one page on your form. Let me show you how:

1. Open your form in Form Builder and select the on the PDF editor on the upper left dropdown menu.
2. In PDF Editor, click on the Add Form Logo on the left side of the page.

• Then drag the logo in the upper right corner.
• Here, you can now resize the logo to fit the page.
