Form Issue: Forms not available upon submit

Estamos sem receber formularios devido a um erro conforme imagem abaixo, que aparece quando é feito a tentativa de envio de formulario, em todos os nossos relatorios. ja foi feita a limpeza do cache e o erro permanece.
