Where can we get our contract with Jotform?

    Com relação ao contrato de utilização, como podemos conseguir? Teria um contato a quem podemos direcionar esta solicitação?

    Hi Samuel,

    Thanks for reaching out to Jotform Support. If you're referring to the Business Associate Agreement, you can request it after you enable HIPAA compliance features in your account from your Account's Data Page. Let me show you where to find it:

    1. Click on your avatar in the top-right corner of the page.

    2. Select Settings from the dropdown menu.

    3. Go to the Data tab and under the HIPAA Compliance Features, you can check the BAA by clicking the View BAA button.
