Problemas no preenchimento do formulário

Olá, boa tarde !

Sou Jayne Marcelo Barros Gomes fou selecionada para participar do time CNT e desde o último dia 19/11 eu encaminhei a documentação e assinei o termo, mas eu perguntei por e-mail se estava tudo certinho e fui informada de que certificado da condição de microempreendedor deveria ser atualizada por estava com o endereço divergente do endereço da instituição estadual e municipal e também deveria enviar um outro comprovante bancário. Mas infelizmente eu não consegui adicionar a documentação pelo link que me foi enviado, não se se é pq já tinha usando para encaminhar a primeira vez a documentação vocês poderiam me ajudar ?
