Now, let me help you with your question. I've checked your form and can see that it only has one submission. I also noticed that your form does not have the Submit button. Please add the Submit button so people can submit their responses. Once they submit the form, you will receive the Notification Email. Let me show you how:

• Click +Add Form Element on the left side of Form Builder.

• Then select the Submit element.
