Há como gerar boleto no plano anual?
Precisamos da nota fiscal também?
Ha possibilidade de colocar no campo do "CPF" do formulário pra ser obrigatório incluir somente resposta com 11 números (XXX.XXX.XXX-XX)?

I will need some clarification of what you are trying to achieve. When you mentioned bill/invoice, is it for the form that you are creating or for the Jotform subscription?
It seems that CPF is sensitive information from the user that might cause your account to be suspended by violating part of the Terms and Conditions shown on this link.

A forma de pagamento via Boleto, queremos pagar o plano anual, mas por boleto.
Também precisamos de nota fiscal.

Or, if you'd rather have support in Portuguese, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.
Now, let me help you with your question. I believe you're referring to Pay by Purchase Order. Unfortunately, Jotform no longer offers the purchase order payment option. We also can't provide an advance invoice, as the invoice will be generated by our system only after the payment for upgrade is done. But, if it's necessary for you to use the method you're looking for, you need to send your request via email to quotes@jotform.com. We can then provide you with an invoice that you can pay either electronically or by mail through a check.
