 I checked the form, and it appears that you have set the Unique Submission to Check cookies and IP address option.

Do note that if you set this option, people in the same network (in the same school, college, university, or institution) will not be able to make submissions, as the IP address for the network will be the same or the same browser. As a result, you get the Sorry! Only one entry is allowed error. Here's how to disable it:

• Go to the Settings in Form Builder.
• Click the Show More Options button.
• Scroll down to the Unique Submission section and set the No Check option to make the form
