Como executar Widget "Planilha para Formulário" em segundo plano?

Não estou conseguindo executar o Widget "Planilha para Formulário" em segundo plano, apesar de fornecer este comando na configuração. Gostaria de ajuda.

Can you please provide us with the error message you're getting?

Make sure that the code you're entering matches with the Unique Code column in the Excel file. Also, you need to use the correct field label in the column in order to pass the data successfully. 

Refer to this guide on how to use the spreadsheet to form widget.

Once we have a better idea of what’s going on, we’ll be able to come up with a solution.
