Uso um fluxo de aprovação nesse formulário, ele está pedindo senha para aprovação, porém, está com a opção desmarcada.
como eu posso proceder?

Now, let me help you with your question. When I cloned and tested your form, everything worked the way it was supposed to, and it did not ask for a password when I clicked approved. Take a look at the screencast below to see my results:
Can you go ahead and try it again and let us know how it goes? If the same thing happens again, can you send us the error message you're receiving? This will help us look into this further. We've got a guide here explaining how to do that.
