Now, I'll help you with your question. I am sorry you are having trouble with this. I cloned your form and tested it to see if I could replicate the issue but everything was working properly. Check out the screencast below to see my results:

If we remove the Square Integration and then integrate it again, that usually solves the issue. Let me show you how:

1. Open your form in Form Builder, and click Add Form element.
2. Click on the three dots in the square under the Payments tab.
3. Now, click on the green Connected button and select Disconnect.
4. Then, click on Connect and connect the Integration again.
5. Click on Continue, and that's it.

Can you try it again and see how it goes? If you run into the same issue again, let us know, and we'll do some more testing to see what's going on.
