Atualização do campo Maiúsculo

Em meu formulário <https://form.jotform.com/bezerraneto/gentedocampo> inserí um código CSS para que apresente no momento da digitção os campos em maiúsculo. Na tabela inseri uma coluna <Nome_M> com a fórmula uppper para inserir os nomes em maiúsculo. Ao utilizar esse campo <Nome_M> nos agradecimentos ele não aparece para inserir. No e-mail recebido também não consta, mesmo inserindo nas configurações de envio. No anexo <pdf> não aparece no nome do arquivo nem no próprio arquivo pdf, mesmo inserido ele nas configurações no Editor de PDF.
