Tenho um site que incorporei o formulário via html, como faço para mandar algumas das informações para outro formulário incorporado?

o que tenho que colocar após o final do link do site para que os dados cheguem no site tem algum exemplo? www.site.com.br/ducumentos/??? o que colocar aqui?

Incorporei via html se tiver uma forma ideal para fazer essa transferência de dados qual seria?
