I just want to confirm, did you fill out a form/App that was created through Jotform? Please be informed that Jotform supplies forms and tools to owners but we are not in control of the services and data they are gathering unless they violate our Terms of Use.

I suggest reaching out to the owner of the form from where you got the form/App that you filled out to inform them about the changes you want to make.
