Estou elaborando um formulário para uso de serviços de estética onde quero escolher uma lista de serviços (29 tipos de serviços) e a medida que escolher, quero descrever em seguida a lista de termos contratuais para os serviços escolhidos. Ex: Marcar os serviços de Massagem Relaxante e Limpeza de Pele de uma lista suspensa. Em seguida, gerar uma lista de descrição destes termos somente destes serviços escolhidos.

Hi Paulo,
Thanks for patiently waiting. You can use a Short Scrollable Terms widget on the form to achieve your requirements. Let me show you how to do it:
- In Form Builder, click on Add Form Element from the top left corner of the screen.
- Go to the Basic tab from the opened window on the left, and drag/drop the Dropdown field into the form.
- Now, you'll see the Properties window on the right. Go to the Options tab and enter your options. Otherwise, click on the Gear icon available with the field to open the Properties window.
- Next, go to the Widgets tab and search for the Short Scrollable Terms.
- Click or drag/drop the widget to your form.
- Now, you'll see the Widget Settings window on the right. Go to the General tab and set up the properties. Otherwise, click on the Wand icon available with the widget to open the Widget Settings window. In Widget Settings, you need to add the following:
- Checkbox Label. The label of the whole widget.
- Content. This is your actual scrollable terms and conditions.
- Must Read all the Text. Check this option if you want users to read all the texts first before they can check the Checkbox.
- Send Content. If you want the terms to be added/included in both the notification and autoresponder emails of the form.
- Finally, click on the Update Widget button.
Now, set up a Show or Hide Fields Based on the User's Answer condition on the form to show the related term's widget on the selection of the related service. Let me show you how to do it:
- In Form Builder, click on Settings in the orange navigation bar at the top of the screen.
- Click on the Conditions from the left panel.
- Click on Add a new condition.
- Select the Show / Hide field condition.
- Add the conditions as shown below in the screenshot and make sure to click Save. Make sure to add conditions for each option.

Vou realizar passo a passo passo o procedimento.
Meu objetivo é descrever os termos de serviços escolhidos, e não todos de uma vez.

But, unfortunately I just receive now the short terms chosen.
It is not coming anymore the previus fields.
How can I fixe it?

Thanks for getting back to us. Can you explain a bit more about the previous fields you are referring to? Have you deleted some fields on the form that you would like to recover on the form again? It would help us understand the issue better if you could share some screenshots showing this and the Form URL you are referring to.
