Oops! Error when adding page break and submit button

Outra coisa, não estou conseguindo finalizar meu formulário porque fica aparecendo esse erro no canto da página, para eu atualizar a página, porém atualizo e continua dando o mesmo erro. Isso acontece com quebra de página e adicionando o botão de 'enviar'. Já tentei de tudo mas não dá certo.
