Form appears as blank when shared

O primeiro erro que dá, quando compartilho o link do formulário, aparece várias páginas como essa em branco. Já exclui várias vezes as condições porém continua dando esse mesmo erro.

Now, let me help you with your question. I tested your form and I was able to replicate the issue. In the first question O seu contrato será em Pessoa Física ou Jurídica?, when I answer Pessoa Física I was taken to a blank page. When I checked the conditions, I see that you set to show certain fields when the condition in the screenshot below is met:

After further checking, the fields that you are trying to show are not set to hidden, so that causes the issue. What you can do is the other way around, when the condition is met, you can hide the elements that you don't want to be displayed.
