Now, let me help you with your question. I do not see the video you provided but only the URL form. It would help us to understand what’s happening if you could take a screenshot and send it to us. We have a guide here explaining how to do that.

Thanks for the screenshots. It seems that the website is not loading properly. When I cloned and tested your form, everything worked the way it was supposed to. Take a look at the screenshot below to see my results:
Can you try it again and see how it goes? I would suggest trying a different browser or try to clear cache and cookies to your browser. If you run into the same issue again, let us know, and we'll do some more testing to see what's going on.

Thanks for getting back to us. I understand that the steps I provided above are not able to fix your concern. It seems that this is a draft form, which means you previously saved and wanted to continue the form, but when I check the draft it says that the draft has been deleted or submitted. Can I ask for the Form URL that you're referring for?
Once we hear back from you, we'll be able to move forward with a solution.
