Problema acessar formulário

Estou com problema para acessar meu formulário (https://www.jotform.com/form/231505179644054)

Pelo computador conseguimos acessar, mas no celular não (conforme imagem abaixo).

Poderia resolver e auxiliar? Utilizamos muito diariamente no trabalho.
