Form User: Haven't received a response from the Form Owner

    Fiz o pedido de uniforme, e não obtive resposta se foi finalizado. E até agora não chegou, só me enviaram um email com meu pedido. Gostaria de saber

You will have to contact the owner of the form by replying to the email notification that you received and telling them that you haven't received any response from the order form that you submitted.
