Why is my form disabled?

    Now, let me help you with your question. I'm sorry this is happening. I went ahead and checked your account and it shows that your account is suspended due to sending emails more than the daily threshold of 10 email invitations.

    In addition to this, if you need to send more than 10 email invitations, you would need to upgrade your plan to a paid one. This will allow you to increase your daily email invitations. You can also check our pricing page to see their price and limit differences between the plans and their features.

    I also checked your form and I saw that it is collecting sensitive information such as Social Security Number while your account email address is in a free domain, which is a violation of our Terms of Use. In order to reinstate the account, we will have to remove the offending fields.
