Problemas na visualização do formulário

Bom dia,

Ontem num chat pedi ajuda. contudo, até ao momento nada me foi dito.

Tenho o formulário [https://eu.jotform.com/build/230314559921354] que em modo "Edição" mostra todas as questões e no modo "Visualização" não mostra a questão 4. certifiquei que não está definida como "Oculto".

Poderão fazer o favor de ajudar a resolver esta situação?
