Como deixar visível a descrição do questionário?

Boa tarde!

Criei um formulário para manutenções e nele, especifiquei o que preciso que seja visto em cada vistoria (usando a descrição do campo "texto longo". Porém quando baixado o arquivo só aparecem as perguntas e as respostas.

É de extrema importância que apareçam também as descrições, há alguma maneira?
