I'm getting 'Form not Found' error to the forms I created.

Now I am trying to use it and it has stopped working.

Thanks for getting back to us. I checked your account and found that it has been suspended by our system because your form collects login credentials, which is against our Terms of Use. I've removed the offending fields in your form and reactivated your account. Your form is now enabled.

Thanks for getting back to us. You can change to something similar like an "access code" but using the words "password" and "login" will trigger our phishing detector to flag your form and suspend your account.
