How to import Word document?

Eu tenho um documento em word que eu faço manualmente.

Gostaria de inserir no Jotform e enviar para um cliente este documento.

Quando o cliente receber o documento, deve existir uma opção "Aceita" ou "Não Aceita"

Se o cliente clicar em Aceita, automaticamente abre um formulário de cadastro criado no Jotform https://form.jotform.com/232084155343653

Now, let me help you with your question. Unfortunately, only PDFs are accepted. You can convert your Word Document to a PDF and then upload it to create a Smart PDF Form. Let me show you how:

• Click the Create Form button on your My Forms page. 
• Choose Import Form and then Import PDF Form.
• Now, upload your PDF file.

Sometimes, some fields are missing. You can add them using the Add Form Element + menu on the left side of the page.

For your form requirement, do you want the users to be redirected to a different link after they submit the form given that they selected the Accept option? If so, you can do that using the Change Thank you Page Condition.
