Não consigo ter acesso mais ao aplicativo

    Boa noite não consigo ter mais acesso ao meu aplicativo da Will bank pois não tenho mais o número que está cadastrado e nem tenho mais a senha de acesso mim ajudem por favor ou se não vou bloquear se vcs não ajeitarem isso.

    Please also note that Jotform is an online Form Builder supplier. If you are referring to a form application, the form is likely created by one of our users. In that case, we suggest you to contact the form creator regarding your application. You can find the contact to the form creator by from where you receive the form URL or where the form was embedded in.
