Now, let me help you with your question. I went ahead and checked your form and I noticed that your Appointment Field isn't set to required, so it is possible that your users skipped that field. You can set the Appointment Field to Required to guarantee that it is filled out. Here's how to do it:

1. In Form Builder, select your Appointment Field.
2. Click the Gear icon to open its Properties.
3. Under the General Tab, go to Required and toggle it On.

After doing this and you still encounter the same issue, let us know so we can look into it further.
