Hi Casa_Fitness,
Thanks for reaching out to Jotform Support. Unfortunately, our Portuguese Support agents are busy helping other Jotform users at the moment. I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in Portuguese, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.
Now, let me help you with your question.
In order to send a response email to the form user in the approval flow, you can add the approval email element in your approval flow from the element section and select the email address field or enter the email address in the email settings.
After this, an email will be delivered to the email address mentioned in the approval element, after the approver approved or denied it.
You can check this guide about How to Set up a Send Email Element in Jotform Approvals
Let us know if you have any other questions.

Hi Casa_Fitness,
Thanks for getting back to us. The comment box when approving or denying an Approval Request is enabled by default. You just need to click the More Actions button in the Approval Request email to go to the Inbox page.
On the Inbox page, you'll then have the option to leave a comment before clicking the Approve or Deny button.
But if you want to require adding a comment first before being able to approve or deny a request, this is also possible and it's quite easy to do. Let me show you how:
1.) From Form Builder, click the small downward arrow button beside the Form Builder text on the top-left part of the page.
2.) Next, click the Approvals icon
3.) Once you're on the Approvals Flow page, click the Settings button (Gear icon) of your Approval element
4.) Go to the Advanced tab and toggle the Require Comments setting to ON
And that's it! Whether the approver will approve or deny the request, he would need to leave a comment first.
Let us know if you have any other questions.
