How to disable password protection on form.

Criei o formulario, quando a pessoa recebe e abre aparece uma caixa om uma senha, e com ícone continue porem não abre.

After checking your form, I have seen that your form has password protection which is why when the user receives your form it displays a box with a password that needs to be entered. To remove this, uncheck the enable password protection on your form builder settings.
