My form doesn't appear complete when someone fills it out.

O meu formulário "ficha de avaliação" não aparece completo quando as pessoas clicar em enviar.

Quando vou ler fica a faltar certas perguntas que são obrigatórias para conseguir enviar o formulário porem não aparece ...

Now, let me help you with your question. Sorry that you're having trouble with this. I cloned your form and tested it to see if I could replicate the issue but everything worked properly. Check out the screencast below:

Can you share with us which field was missing from the submissions? It would help us to understand what’s happening if you could take a screenshot and send it to us. We have a guide here explaining how to do that. Also, I have cleared your form cache as an extra measure. Can you try it again to see how it goes?

Once we hear back from you, we can move forward with a solution.
