Gostaria de replicar no Jotform um Formulário de Parte Diária de Equipamentos

Já faço uso de um Formulário de Parte Diária de Equipamentos físico, que é um formulário mensal, mas que é preenchido manualmente todos os dias.

Esse formulário segue anexo abaixo. Gostaria de ajudar para replicá-lo no Jotform.

Now, let me help you with your question. The attached document can't be replicated exactly how it looks even in Form Builder or using the Smart PDF Editor. In this case, using the Smart PDF Editor, upload it as a PDF and let the system create an online form field. If there are missing fields, you can add them manually.

Through the Smart PDF Editor, you can let the responder enter their response to the form fields, although the online form counterpart doesn't look like the original PDF, the response will still be mapped to the original PDF.

Open this guide on how to upload the PDF and create an online form out of it. Now, once uploaded and you need to create a form, you have to map the fields to the PDF so the response in the online form will display in the PDF.
