Seeing incorrect form title when the form is sent out to a user

Bom dia,

Tenho um formulário configurado que está com o título Unimed odonto 2023, mas quando o formulário é enviado para preenchimento aparece como Unimed odonto 2022.
Gostaria de saber como faço para corrigir esse problema.

After checking further into your account, I see that it's the Unimed Odonto NIE Jaú 2023 form that is probably being sent out where it shows the following text when opened:
'Unimed Odonto 2022
Preencha este formulário para aderir ou excluir seu plano odontológico UNIMED BH.'
