Não está sendo possivel anexar o link de avaliação

Ao tentar anexar o link de avaliação, o site retorna informando que o formato está incorreto.

Or, if you'd prefer to receive support in Portuguese, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.

As for your concern in regard to the embedded form link to your webpage, can you please tell us if the link was created outside Jotform? If you are trying to add the form to your webpage, please note that you will have to copy the form's direct link which can be found in the Publish tab:

You can also embed the form into your webpage using its embed code.
