    Thanks for reaching out to Jotform Support. I would like to inform you that I will be using translation tool in order to provide assistance. If you would like to receive response in your native language, we can forward your ticket to our Portuguese Support, however, it may take a while to get a response.

    Moving forward with your concern, I checked the account and it is currently suspended because one of the forms is collecting Login Credentials which is against our Terms of Use. I deleted the fields and reactivated your account.
