Thanks for getting back to us. I checked your form to see if I could replicate the issue, but everything was working properly. In console logs, it does not show an error about JFCustomWidgetUtils anymore. Check out the screenshots below to see my results:
Can you try it again and see how it goes? I have cleared your form caches as a precaution to avoid temporary cache issues. Could you please clear your browser's cache as well? If you run into the same problem again, let us know and we'll do some more testing to see what's going on.Reach out again if you have any other questions.
