Now, let me help you with your question. I’m sorry you're having difficulties with this. Let me help you set that up. Are you referring to the Page Title? If so, it is easy to change the Page Title on your form. Let me show you how:

1. In Form Builder, click on Settings in the orange navigation bar at the top of the page.
2. Click on Show More Options.
3. Scroll down, enter the Page Title you want to use. and that’s it. You’re done.
