Criação de subdominio para o cloudflare

Prezados , Bom dia ! 

Sou Luciano do setor Cpd da OAB-BA  

Recebi uma demanda para inclusão de um sub domínio (form.jotform.com/PRERROGATIVASBA/Online)

no nosso cloudFare, porem estou tendo um problema de incluir o recurso (PRERROGATIVASBA/Online) pois o mesmo não aceitar entrada cname devido ao recurso  (PRERROGATIVASBA/Online)

tendo em vista que o que mostra para é somente o ip publico (35.201.118.58).
teria como nos ajudar com um informativo de como criar este subdominio ?

form.jotform.com is our domain that resolves to 35.201.118.58. PRERROGATIVASBA/Online is redirected on our server to the form.
