Como duplicar as mesmas perguntas no mesmo formulário

Estou criando um formulário para cadastro de clientes e seus pets, minha dúvida é, se o tutor ter mais de um pet para preencher as informações, consigo tipo que duplicar as mesmas perguntas em um mesmo formulário?

Thanks for getting back to us. It's easy to show the hidden fields on the form. Let me show you how:

1. Go to the Settings from the top navigation bar.
2. Click Conditions from the left panel.
3. Click on Add a new condition and select Show / Hide field.
4. Add the conditions as per your requirements (see an example in the screenshot below).
5. Make sure to click Save.
