Thank you page: Unable to add download PDF button

Não estou conseguindo adicionar um botão para download em PDF na página de agradecimento do meu formulário "Relatório de Avaliação Mensal", poderiam me ajudar?

With that being said, I've tried to add the download PDF Button on your form but it worked fine for me. Please make sure to click the Select PDF dropdown and choose the default or your customized PDF. Once selected, it should allow you to click the create button, that's it!
