Problema desconhecido no formulário

Olá, estava fazendo um formulário, e estava funcionando normalmente, quando, do nada, ele ficou com problema, que não sei como resolver. E não posso perder as configurações já feitas no formulário.

Na parte de edição do formulário, ele está normal. Somente via link que ele apresenta bug.

When I cloned your form, I noticed that the Categoria do Curso field would disappear after being filled out:

O problema era que, o formulário estava todo com bug, creio que por ter as condições divergentes. Porem reparei que voltou ao normal, graças a sua ajuda. Obrigado.
