Form can't be filled out

As for your issue, I cloned your form and tested it to see if I could replicate the issue, and I ran into the same issue. When I looked at your form, I saw that the theme of your form is causing the issue. To fix the issue, you can change the theme of your form. Let me show you how to do it:

1. In Form Builder, click on the Paint Roller icon on the upper right side of the page.

2. Click on the Themes tab.
3. Click on Use Theme of the theme that you want.
