como criar perguntas condicionais?

quero criar um relatório, que tenha perguntas condicionais.

Quero colocar uma caixa com 5 opções de Região, e dependendo da região que a pessoa selecionar, irá abrir as opções de cidades daquela respectiva região.

o restante eu ja tenho uma idéia. poderia me ajudar?

Now, let me help you with your question. You can add All States and Cities widget to your form to automatically select a city and state according to the selected country. Let me walk you through it:

1. Open your form in Form Builder, click on the Add Form Element menu on the left side of the page.
2. Click on Widget tab, search All States and Cities, and drag and drop it on the form.
