Do you want to combine the unique ID with the contact number of the person who filled out your form? If yes, this is possible. Let me show you how:

1. Add a short text field in your form and set it as hidden.
2. Create a condition to combine the unique ID and the phone number field if the phone number field is filled, then pass it to the short text field.

a. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.

b. Click on Conditions on the left of the page.

c. Now, click on Add a New Condition and then click on Update/Calculate Field.

d. Set the conditions like you see below:
