1) Como altero o nome do formulário?

2) Estou enviando o formulário p os clientes e eles não conseguem me enviar de volta pois não tem nenhum botão

As for changing the form name, you should be able to do it in your account settings. Let me show you how:

1. On your form builder page, go to Settings.

2. You can then change the Title to your desired form name.
