Our Portuguese Support agents are busy helping other Jotform users right now, so I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in Portuguese, let us know and we can have them do that. But, it might take a while until they're available again.
Now, coming back to your question, I checked your account and didn't find any forms. Are you referring to this form from another account? If so, and you are not a HIPAA Covered Entity can you log out of your account, then clear your browser history and log back into the account that owns the form. You should be able to see the modal below. Then do the following:
- Click the Go Back button, and you should see another modal for the next step.
- In the following modal, select the NO, I'm not a HIPAA-covered entity option.
Once you select this option, the forms will be enabled. However, it may take a few minutes to take effect.
