Upload de arquivo: Ocorre um erro ao enviar o formulário com o elemento de upload de arquivo

Boa tarde

Estou tendo problema com os formulários ao tentar realizar o preenchimento, eles estão retornando alguns erros conforme os prints abaixo

Neste mesmo formulário (https://submit.jotform.com/232915295252660) ao tentar anexar um e-mail ele esta retornando este erro.

Estes são os erros deste formulário, mas outros usuários ja me reportaram problemas similares em outros formulários, poderiam verificar se existe algum problema com a ferramenta.
